Transplant immunology I: mechanisms of rejection in solid organ transplants

چکیده

Non-identical transplants (allogeneic) have been a modern medicine milestone; however, the major pitfall for completed succeed is recognition of foreign organ by immune system. The myriad variations in sequence molecules histocompatibility complex (MHC) or human leukocyte antigen (HLA) between individuals are cause allograft rejection. Thus, B T lymphocytes receptor occurs hypervariable regions HLA that triggers to graft 3 types rejection: hyperacute, acute, and chronic. In addition, rejection could be antibody-mediated (ABMR), T-cell-mediated (TCMR), both, which depends on main branch This review will explore mechanisms mentioned above give an introductory insight into transplant immunology; thus, preparer reader delve further reviews Histocompatibility Immunogenetics laboratory purpose.

Noninvasive diagnosis of acute rejection of solid organ transplants.

Accurate diagnosis of acute rejection remains a formidable challenge in organ transplantation. The current gold standard diagnostic test for acute rejection is histological examination of the transplanted organ. However, biopsy procedures are invasive and complications occur. Furthermore, sampling errors may bias the histological diagnosis. Not uncommonly, empiric anti-rejection therapy has to ...

Universal noninvasive detection of solid organ transplant rejection.

It is challenging to monitor the health of transplanted organs, particularly with respect to rejection by the host immune system. Because transplanted organs have genomes that are distinct from the recipient's genome, we used high throughput shotgun sequencing to develop a universal noninvasive approach to monitoring organ health. We analyzed cell-free DNA circulating in the blood of heart tran...

Tolerance in Solid-Organ Transplant.

Immunotolerance, which is nonimmunologic reactivity to specific tissue, was demonstrated in animals in the 1950s. However, despite assiduous efforts, it has not been reproduced in human solid-organ transplant to date. Fortuitously, clinical operational tolerance, which is stable graft function for > 1 year with no immunosuppression, has been demonstrated, primarily in a few nonadherent recipien...

Nocardiosis in Solid Organ Transplant

The genus Nocardia includes more than 80 known species, with at least 33 of them related to human pathology [2,3]. It is an opportunistic pathogen, causing infection generally in immunosuppressed, mainly with long exposure to corticosteroids, transplanted or HIV positive patients. It has worldwide distribution and is very ubiquitous, being found in terrestrial and aquatic environments as well a...

The Swiss Transplant Cohort Study's framework for assessing lifelong psychosocial factors in solid-organ transplants.

BACKGROUND Understanding outcomes after transplant requires a biopsychosocial model that includes biomedical and psychosocial factors. The latter, to date, are assessed only in a limited way as part of transplant registries or cohort studies. The Swiss Transplant Cohort Study (STCS) is a nationwide open cohort study (starting May 2008) to systematically and prospectively assess psychosocial fac...
